Physician’s CERTIFICATION fee is $150 if paying with Venmo App or $155 by credit/debit card (this fee is waived for US Veterans). Patients who NO SHOW for their appointments will be charged an additional $25 if they choose to reschedule

Payment to the State Department of Health is separately paid on their website ($50; free if on government assistance)

All certifications valid for one year
