背景介绍: 促甲状腺激素(TSH)是垂体前叶分泌的一种分子量为28KD的糖蛋白激素。促甲状腺激素有两个亚基,即α和β。促甲状腺激素的α亚单位与LH、FSH和α糖蛋白激素中的hCG亚单位相似。促甲状腺激素(TSH)正向刺激甲状腺激素T4和T3的产生。循环T4和T3通过负反馈调节TSH分泌。促甲状腺激素(TSH)的产生也受到下丘脑分泌的促甲状腺激素释放激素(TRH)的积极控制。血清TSH的测定被认为是诊断原发性和继发性甲状腺功能减退症最敏感的指标。在原发性甲状腺功能减退症中,甲状腺激素的产生受到损害,TSH水平通常会很高。在因垂体或下丘脑病变而导致甲状腺激素水平低下的继发性或特发性甲状腺功能减退症中,TSH水平通常较低。此外,敏感的TSH检测也能区分甲状腺功能亢进症和正常甲状腺人群。在大多数甲状腺功能亢进症中,TSH通常被抑制到低于正常水平。建议同时测定TSH和甲状腺激素,作为甲状腺功能状态的临床评价指标。但如果只有一项甲状腺功能测试,那么TSH就是一种测试。TSH测定也有助于监测接受甲状腺素替代治疗的患者。 Thyroid stimulating hormone (TSH) is a glycoprotein hormone of 28KD secreted by the anterior pituitary gland. TSH has two subunits, namely α and β. The α subunit of TSH is similar to the α subunit found in the LH, FSH and hCG glycoprotein hormones. The β subunit however, is specifi c and differs from hormone to hormone. TSH stimulates positively the production of thyroid hormones T4 and T3. Circulating T4 and T3 regulate the TSH secretion by negative feedback. TSH production is also under the positive control of thyrotropin-releasing hormone (TRH), which is secreted by hypothalamus. Measurement of serum TSH is generally regarded as the most sensitive indicator available for the diagnosis of primary and secondary hypothyroidism. In primary hypothyroidism, where there is impaired production of thyroid hormones, the TSH level is typically highly elevated. In secondary or tertiary hypothyroidism where the thyroid hormones are low as a consequence of pituitary or hypothalamic lesions, the TSH level is usually low. Further, a sensitive TSH assay is also able to differentiate the hyperthyroidism from the euthyroid population. TSH is typically suppressed to subnormal levels in most hyperthyroidism. It is recommended to assay both TSH and thyroid hormones for the clinical assessment of thyroid status. But if there were to be only one test to be prescribed for thyroid function, TSH would be the test. TSH determinations are also helpful to monitor patients who receive thyroxine replacement therapy.
