The film is inspired by the true story of a tour of the Deep South by African American classical and jazz pianist Don Shirley and Italian American bouncer Frank "Tony Lip" Vallelonga who served as Shirley's bodyguard.

The film was written by Farrelly, Brian Hayes Currie and Vallelonga's son, Nick Vallelonga, based on interviews with his father and Shirley, as well as letters his father wrote to his mother. The film is named after The Negro Motorist Green Book, a mid-20th century guidebook for African-American travelers written by Victor Hugo Green.

The Negro Motorist Green Book (also The Negro Motorist Green-Book, The Negro Travelers' Green Book, or simply the Green Book) was an annual guidebook for African-American road- trippers. It was originated and published by African American, New York City mailman Victor Hugo Green from 1936 to 1966, during the era of Jim Crow laws, when open and often legally prescribed discrimination against African Americans especially and other non-whites was widespread. Although pervasive racial discrimination and poverty limited black car ownership, the emerging African-American middle class bought automobiles as soon as they could, but faced a variety of dangers and inconveniences along the road, from refusal of food and lodging to arbitrary arrest. In response, Green wrote his guide to services and places relatively friendly to African-Americans, eventually expanding its coverage from the New York area to much of North America, as well as founding a travel agency.

The film has been criticized for advancing a white savior narrative in film that perpetuates stereotypes. Salon said the film combines "the white savior trope with the story of a bigot's redemption". He said he had long discussions with the actors and producers on the point, and believes that it was not advanced by the film, saying it is "about two guys who were complete opposites and found a common ground, and it's not one guy saving the other.
